Waivers, Programs and Services Offered by SCU

Service Coordination Unlimited, Inc. (SCU) coordinates services for people with physical disabilities, intellectual/developmental disabilities, autism and older adults helping them live independently in their homes and communities. We work with various state programs and waivers using a pereson-centered strategy to determine individual needs and prefernces.

This commitment results in better outcomes for individuals and a more fulfilling work experience for staff.  SCU coordinators build strong relationships with participants, assisting them in creating, implementing and monitoring a person-centered care plan.  The overarching goal is to ensure each individual leads a safe and meaningful life within their community. Our holistic approach empowers our participants to make choices about their own care, ensuring they receive the necessary supports to maintain their independence and dignity.
