Community HealthChoices — Managed Care Program for Pennsylvania’s Aging and Disabled
Community HealthChoices (CHC) is a mandatory managed care plan in Pennsylvania that was developed with the intent to improve coordination of physical and behavioral health systems, as well as Long Terms Services and Supports (LTSS).
Community HealthChoices aims to improve the quality of in-home supportive services for people who require long term care and wish to avoid becoming institutionalized. The program offers a wide array of services to support adults with physical disabilities, as well as the aging population in PA, with their choice to continue to live independent lives. CHC is also designed to assist nursing home residents with transitioning back to the community.
All eligible individuals have to choose a Managed Care Organization (MCO) to coordinate their CHC care plan.
